Helping patients take control of their health history data

Gone are the days of having to fill out your patient information with a pen and a paper. We partnered with Pacific Dental Services and Talosity Digital to create a health history web application so patients can fill out their information online prior to their appointment, and they can keep their dentist up to date on any changes in their health. The new application adapts the questions based on your responses, so you only have to fill out the questions that are pertinent to you, and it connects directly with their back end systems so they get updated information in office. Complete with the ability to sign online, this form is HIPPA compliant and secured to the highest standards to ensure all patient information is kept confidential.
